How to Enroll

Enrollment is now open for 2012-2013 school year.

Begin the process of enrolling your student(s) by following the steps below.

New Families set up a user account

Have you already set up an account? Continue your enrollment or check your student’s enrollment status.

Your child must be 5 years of age before September 1, 2011 to enroll.

Students with IEPs must reside in Provo School District.

After you have completed the online application, download (where applicable) and submit the required forms:

• Enrollment Forms Packet
  
• Proof of Utah Residency (i.e.  Utility bill in the name of the legal guardian)
  
• Birth Certificate
  
• Immunization Record
  
• Withdrawal Documents from previous school (if applicable)

Once you submit your application, you will be assigned an individual who will assist you through the enrollment process, answer all of your questions, and conduct your admissions conference over the phone. During this conference, we’ll verify your student's information, relay school policies, and review the responsibilities of both the student(s) and the parent(s).


Upon receipt and successful review of submitted documents your student will be approved. You may review your student’s enrollment status by logging in to your online account.
